fix: prioritize exact substring matches over fuzzy in model search
...
- Exact substring in label (earliest position wins)
- Word-boundary prefix matches
- Description substring matches
- Fuzzy matching as fallback
This ensures 'opus' shows claude-3-opus before openrouter models.
